RECORD REQUESTS

 

If you are a patient and need copies of your medical record, we encourage you to access your patient portal.

If you need paper or electronic copies, please call our medical records department at 217-324-1040.

Please note IL medical record fees:

​

Pages 1 - 25: $1.09 per page if paper copy or $.55 per page if CD
Pages 26 - 50: $0.73 per page if paper copy or $.37 per page if CD
Pages 50+: $0.36 per page if paper copy or $.18 per page if CD
Microfilm or other media: $1.82

Handling Fees for Third Party Requests: $27.95 for paper copies and $13.95 for CD

Worker’s Comp:
Subpoena for records : $20.00 flat fee

Electronic Requests: $6.50

Our Medical Records Department will get records out within 15 days whenever possible.
