Please present your current insurance card each time you visit the office.  By doing this, you can assure that your insurance claims will be processed timely and accurately.


Litchfield Family Practice Center uses state-of-the-art computer technology to verify eligibility and to electronically file your office, hospital, and laboratory charges. Showing your insurance card every time you visit may seem like extra work, but insurance coverage and companies can change, and we want to ensure we are giving you the best service possible when processing your visit.

Plan benefits vary according to the insurance plan purchased personally or by your employer.  Please be aware of what coverage is available to you and which services require approval by your insurance company.  If you have difficulty understanding your benefits, we encourage you to bring in your plan document and we will assist you or direct you to whom you should call at your insurance company.  You can contact the LFPC Insurance Department directly at 217-324-1041.

If your plan requires pre-certification prior to a procedure, our insurance staff can assist in handling the paperwork.  To ensure payment, all plan provisions must be completed prior to your procedure. If you have any questions about pre-certification, please call or come in to speak to one of our staff members.

Your co-payment will be collected prior to being seen by your provider.  If you have a plan that requires a percentage of the office charge, that amount will be expected at the end of your visit.

If you have a high-deductible health plan, we do require a $50 payment prior to being see for a clinical visit and $100 payment prior to being seen for a mental or behavioral health visit.

You will receive a monthly statement for the balance on your account that is not covered by insurance or other coverage. We accept cash, check, money order, and major credit cards.

Payment is expected upon receipt of your statement, but our financial services staff is available via phone or in person during normal office hours to discuss payment arrangements.  You can contact Cassie at 217-324-1047 or Rachael at 217-324-1046 to discuss details.
